Program Overview – Homeward Bound & Neighbourhood Programs
The program includes components such as career development, life skills, academic upgrading, computer training, and a college diploma program. It also offers internships and access to sustainable employment opportunities. Additional supports include on-site childcare, family support, and an after-4 program for school-aged children.
What You Will Do
- Utilize professional case management practices and clinical modalities to perform duties including assessment, goal setting, action plan development, ongoing intervention, re-assessment, case termination, and follow-up.
- Coordinate client participation in the Homeward Bound program, liaising with Community Colleges and other providers to ensure enrollment and participation in programs and internships.
- Ensure clients meet program goals and milestones.
- Coordinate with Toronto Children Services and Toronto Employment and Social Services.
- Assist women in resolving daily living issues, advocate, and facilitate access to health care, financial management, and housing support.
- Identify learning barriers and refer participants to appropriate support sources.
- Respond to emergencies, provide crisis intervention, and maintain a caseload.
- Maintain client records in accordance with program policy.
- Collect statistical data and assist in overseeing safety during off-site trips and on-site activities.
- Work in compliance with relevant legislation, policies, and organizational standards.
What You Bring to the Team
- Masters of Social Work or related Master's Degree; Bachelor's Degree with relevant experience may be considered.
- Registration with the Ontario College of Social Workers and Social Service Workers is required.
- Minimum two (2) years of related experience working with women facing homelessness, violence, or similar issues, including case management and employment planning.
