The Care Navigator serves as a support to families and their treatment teams ensuring that a family's health needs are addressed and that the family is connected to appropriate resources in their community. A Care Navigator develops a Plan of Care for all families in their program and ensures that the plan goals are completed while the family is active with NYF preventive services. Responsible for the overall management of the Family’s Plan of Care. The Care Navigator Work's with their team to help coordinate the enrollee’s provision of services according to their health needs. Seekout and make referrals to preventive health care providers to address the family needs such as.Child wellness visits as per developmental guidelines, evaluations and follow up treatment for dental, vision and hearing care, following Medicaid EPSDT guidelines.
