Under the supervision of the Manager and Care Management Supervisor, this position collaborates with the Integrated Care Team to provide effective care management to members. The role includes outreach, engagement, behavioral health interventions, care coordination, resource linkage, and facilitating seamless transitions of care, all aimed at ensuring quality and timely care for members. The position also involves working with members, families, community professionals, and the healthcare organization's medical team, promoting whole health principles and health education within the community.
Major Functions (Duties and Responsibilities)
- Manage a caseload of members with medical needs.
- Utilize brief medical interventions to enhance members' self-management of health.
- Train and support team members on medical conditions, treatments, and evidence-based practices.
- Assist with care coordination, including:
- Conduct comprehensive assessments within the RN scope.
- Develop individualized care plans (ICPs).
- Communicate ICPs effectively with members, families, and care teams.
- Lead interdisciplinary care team meetings.
- Coordinate with internal and external health partners.
- Participate in and lead care transition planning.
Review and update assessment and care planning contributions from team members.Model ethical behavior and promote a collaborative, respectful work environment using evidence-based communication strategies like Motivational Interviewing.Build and maintain positive relationships with providers through various communication methods.Engage with members in person and by phone, fostering collaboration and promoting health empowerment using evidence-based approaches.Contribute to quality improvement initiatives, addressing HEDIS gaps and testing new practices.Coordinate medical and behavioral health access with providers and ancillary services.Ensure documentation complies with regulatory and accreditation standards.Participate in meetings, trainings, and other activities as directed.Education & Experience
- Minimum two (2) years of clinical experience in acute care, skilled nursing, home health, or clinics.
- Active, unrestricted RN license issued by the California BRN.
- Two (2) or more years of care management experience preferred.
- Experience in Managed Care or acute facility care management preferred.
