Job summary

Ontario Health atHome seeks a Care Coordinator (RN, PT, OT or SW) to assess patient needs, coordinate services, and connect patients with community resources. This full-time role focuses on Parry Sound and home-based, patient-centered care delivered by a collaborative team of professionals.

You will work in a setting that supports professional growth, work-life balance, and high-quality care delivery while engaging with patients, families, and a broad network of health care partners.

Qualifications

  • College licenses required to practice in Ontario.
  • Recent experience in community health or related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment and decision-making skills.
  • Strong time management and organizational abilities.
  • Ability to work independently and in a busy multidisciplinary environment.
  • Proven documentation and reporting abilities.
  • Valid driver’s license and access to a reliable vehicle.
  • Advanced oral and written proficiency in English.
  • COVID-19 vaccination proof as a condition of employment.

Responsibilities

  • Assess care needs and determine eligibility for services and develop care plans.
  • Link patients with service providers and coordinate care delivery.
  • Monitor care plan progress and adjust as needed.
  • Establish relationships with patients and families.
  • Balance patient needs with available resources, respecting values.
  • Initial assignment to hospital with possible changes based on needs.

As a Care Coordinator (Status: Full-Time, Start date: Immediately; Location: Parry Sound -West Parry Sound Health Centre) , you will assess and determine patient careneeds and eligibility, provide access and referrals to community services, andengage with patients, caregivers and other health care practitioners.

Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.

What will you do?
  • In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
  • Linkpatients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs andchoices with available resources, ensuring patients’ values and preferences arerespected
  • Initial assignment Hospital, withpossibleassignmentchanges within the occupation based on operational needs
What must you have?
  • College of Nurses of Ontario
  • College of Physiotherapistsof Ontario
  • College of OccupationalTherapists of Ontario
  • Ontario College of SocialWorkers and Social Service Workers
  • Recentexperience in community health or a related field
  • Knowledgeof the health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • Avalid driver’s license and access to a reliable vehicle
  • Advance oral and writtenproficiency in English is essential
  • We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.
What would give you the edge?
  • Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
  • Casemanagement experience or recent related community experience
  • Abilityto speak French or another second language
Salary:

Hourly wage of $41.29 to $51.13
