Overview

Abbe Center for Community Mental Health is seeking a full time Care Coordinator for our Integrated Health Home (IHH) program in Independence office. Care Coordinators work with a team of professionals to enhance the physical and mental well-being of clients in the IHH program. This position includes Integrated Health Home hours that are office-based and work in client's homes in the Indepedence area. Monday-Friday daytime hours with possibility of a limited work-from-home schedule and flexibility in hours.

Responsibilities

Is aware of department compliance requirements for federally funded healthcare programs (eg. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of department or administrative staff.
Purposefully conducts all aspects of the job in an ethical manner in support of the organization's commitment to ethical behavior in all areas of personal and professional activity.
Demonstrates a positive cooperative attitude, flexibility, and a willingness to change to meet department goals.
Responsible for being well-organized and energetic with exceptional interpersonal and communication skills.
Duties include focusing on the whole health of individuals-both physical and mental health, writing assessments of needs and goals, making referrals, coordinating care and working in a team environment.
Performs additional duties as may be assigned to ensure complete service to all customers.


Qualifications

Education

Requires a Bachelor's degree in social work or human services field.

Qualifications/Experience

Minimum of one year experience working with individuals with mental illness. Iowa Driver's License Required
