Job summary

Ontario Health atHome in Whitby is seeking a Care Coordinator (Temporary Full-time) to assess patient needs, determine eligibility, and connect patients with community resources. You will collaborate with caregivers and health professionals to provide accessible, patient-centred care in a multidisciplinary setting.

The role supports home- and community-based care across Ontario Health atHome teams, with emphasis on documentation, travel within the Ontario Health atHome Central East region, and

Qualifications

  • Must be registered in Ontario with the applicable regulatory body (RN/PH/OT/RSW).
  • 2+ years of recent experience in community health or a related field.
  • Knowledge of health care delivery and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Effective time management, prioritization and organizational skills; able to work independently and with others.
  • Proficient in completing documentation, reports and forms.
  • Valid driver’s license and access to a reliable vehicle.
  • COVID-19 vaccination status is required prior to start date.

As aCare Coordinator, you will assess and determine patient care needs and eligibility, provide accessand referrals to community services, and engage with patients, caregivers and other health care practitioners.

Please note that this position is located in Point of Access, Hospital, the initial assignment is Team Care Coordinator.

What will you do?
  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
  • Link patients with service providers

  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
What must you have?
  • Membership, in good standing, with the applicable regulatory body:
    • College of Nurses of Ontario (RN)
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers (RSW)
  • 2+years of recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s licence and access to a reliable vehicle
  • Proficient in a Windows environment
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
What would give you the edge?
  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
Hours of work:

All hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.

Position location and travel:

Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
