Job description

Reporting to the Care Manager, the AHN is responsible for the day-to-day monitoring of health status and co-coordinating of resident care activities in the LTC setting. Your knowledge and skills, advanced judgment, and clinical experience will assist in providing solutions for complex healthcare issues at all levels with patients, inter-professional teams, administrators, and policymakers.

Responsibilities:
  1. Coordinates safe and seamless professional care across the continuum on the nursing unit.
  2. Assumes responsibility for daily staffing needs and assigns staff accordingly for resident care as required.
  3. Ensures accurate and timely communication with multidisciplinary team members regarding changes to resident status, plans of care, and Physician orders.
  4. Coordinates admissions, discharges, and transfers.
  5. Participates in multidisciplinary conferences to coordinate and prioritize resident care activities.
  6. Engages in quality improvement and risk management activities related to care and service provision.
  7. Identifies and communicates issues requiring follow-up by the Care Manager.
  8. Promotes positive relationships among staff, residents, family members, managers, physicians, outside agencies, and the public.
  9. Participates in staff development and identifies staff education needs.
  10. Assists in setting objectives, planning, organizing, implementing, evaluating, and delivering services on the unit in accordance with standards, policies, and procedures.
  11. Ensures the accurate and timely completion of RAI/MDS assessments and care planning.
  12. Monitors staff adherence to policies and procedures.
  13. Ensures cost-effective and efficient use of resources.
Additional Responsibilities:

As an Assistant Head Nurse, you will be responsible for staff assignments, patient flow, and acting as a role model, clinical resource, and advocate for nursing staff. You will provide frontline nursing care as needed and take a leadership role in promoting quality patient care and implementing operational priorities. Your in-depth knowledge, skills, advanced judgment, and clinical experience will aid in solving complex healthcare issues at all levels with patients, interprofessional teams, administrators, and policymakers. You will play a key role in providing safe, quality, patient- and family-centered care while reflecting the shared vision and values of AHS.

Qualifications:
  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
  • Minimum of 5 years' experience in an LTC setting.
  • Experience working with RAI, including assessment, outputs, and quality indicators.
  • Experience with Care Planning.
  • Knowledge of Connect Care and Microsoft Office.
  • Proven ability to work harmoniously within an interdisciplinary team.
  • Ability to work independently and as part of a team.
  • Effective verbal and written communication skills, with delegation abilities.
  • Strong organizational, leadership, and judgment skills.
  • Commitment to mentoring others.
  • Solid understanding of CCHSS, Accommodation Standards, and audit processes.
Desirable Qualifications:
  • BScN degree.
  • Connect Care education, including Continuing Care Facility Living Nurse and Care Planning Badge.
