Job summary

Providence Healthcare in Toronto is seeking a Clinical Resource Coordinator for our Adult Day Program, which operates 365 days a year with flexible hours and caregiver relief.

As part of an interdisciplinary team, you will deliver direct care and coordinate services, while planning recreational activities for clients with moderate to severe dementia in a community-based setting.

Qualifications

  • Minimum 1-3 years' experience with individuals with Alzheimer's Disease or related dementias.
  • Degree or Diploma in a Health-related field such as Activation, Therapeutic Recreation, Social Service Worker, Gerontology, or Occupational/Physical Therapy Assistant
  • Experience in planning and implementing therapeutic recreation programs and activities, activation methodologies, principles and practice
  • Experience in providing personal grooming, bathing, toileting and nutritional assistance for clients with an irreversible dementia
  • A thorough understanding and working knowledge of Alzheimer's disease and related dementias
  • Trained in dementiaAbility©, Positive Approaches© to Care and/or U-FIRST/PIECES© preferred
  • Demonstrated ability to manage a high volume client caseload and varied work assignments with accuracy, flexibility and in a timely fashion
  • Ability to work independently under pressure and establish priorities under time constraints
  • Self-motivated with demonstrated ability to function as a part of a team and/or independently, creative, flexible and exhibits a good sense of humour
  • Computer literacy with developed skills in various software applications including word processing skills (i.e. Windows 2000 and MS Office), spreadsheets, databases, and presentation/graphics
  • Strong organizational skills, problem-solving skills and critical thinking skills
  • Excellent oral, written and interpersonal and communication skills
  • Knowledge of the Home and Community Care Support Services processes and community services
  • Strong sense of caring and compassion
  • Experience in a community-based setting preferred
  • Strong organizational and communication skills
  • Knowledge and sensitivity to work in a culturally diverse environment
  • Excellent past performance and attendance record.

Responsibilities

  • As a member of the Adult Day Program interdisciplinary team, the Clinical Resource Coordinator is responsible for providing direct service-delivery for our clients who attend the Adult Day Program, through service coordination, personal care and recreational programming, in accordance with Providence Healthcare's Mission and Values.

Are you a dynamic, enthusiastic, flexible, creative individual with a Degree or Diploma in a Health-related field such as Activation, Therapeutic Recreation, Social Service Worker, Gerontology, or Occupational/Physical Therapy Assistant?

Do you have experience working in a Community-based setting?

Do you have experience in developing and leading recreational programming for clients with moderate to severe Alzheimer's Disease and/or related dementia?

Do you have the skills, expertise and knowledge to successfully interact, engage and provide personal care for clients with moderate to severe Alzheimer's Disease and/or related dementia?

Do you have formalized training in Alzheimer's disease and related dementias?
