Position Summary

The ACT Nurse assesses, identifies, plans, implements, and evaluates the care required to assist ACT team clients in meeting their physical, psychiatric, social, and spiritual needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client-centered and strengths-based care in accordance with the BCCNM Standards of Practice and consistent with the mission, vision, values, policies, and protocols of RainCity Housing and the ACT team.

Many clients are certified under the Mental Health Act; knowledge of this Act and its implications is essential.

The Nurse provides treatment services that are inclusive and flexible for individuals with mental illness, addictions, and/or chronic medical conditions, ensuring integration and continuity of care. Responsibilities include intake, mental and physical health assessments, treatment planning, crisis intervention, hospital consultation, wound care, and medication management.

Job: Community Outreach Nurse, Assertive Community Treatment Team, Permanent, Full-Time, 1 FTE

Hourly Range: Starting at $41.42 (BCNU Level 3 RN/RPN)

Hours: 75 hours bi-weekly

Schedule: Rotating 8-week schedule; shifts from 08:30 AM to 6:30 PM, including weekdays and weekends, with a 30-minute unpaid meal break.

Start Date: As soon as possible

Requirements: Clear BC Vulnerable Sector Criminal Record Check; valid Class 5 driver’s license or use of personal vehicle for outreach across Vancouver.

Program Summary

ACT is an evidence-based, transdisciplinary mental health team providing outreach support to clients with severe mental health and substance use diagnoses, emphasizing self-determination, choice, and harm reduction within a housing first framework.

Qualifications

  • Bachelor’s Degree in Nursing (RN or RPN).
  • Current practicing registration with BCCNM.
  • Three years’ recent experience with mental health and addiction treatment or equivalent.

Knowledge and Abilities

  • Knowledge of mental health and substance use diagnoses and resources.
  • Experience with harm reduction principles.
  • Understanding of relevant legislation, including BC Mental Health Act.
  • Awareness of oppression and marginalization issues.
  • Ability to work with diverse backgrounds, independently and in teams.
  • Physical capacity for outreach duties, including light lifting and community mobility.
