peopleCare Communities in Waterloo, Ontario, is seeking an Accreditation Project Manager to lead organization-wide readiness for the Accreditation Canada survey across long-term care homes. Reporting to the VP Quality, Research, and Strategic Partners, you will coordinate plans, evidence, gap closure, staff readiness, and survey activities.

This part-time, 3-days-per-week role runs Sept 2026 to May 2027, with a goal of achieving full accreditation during the May 2027 survey.

Qualifications

• Minimum 5 years of leadership or project management experience in a healthcare setting.
• Experience supporting accreditation, quality improvement, compliance, or complex healthcare projects.
• Working knowledge of Accreditation Canada standards, survey processes, evidence expectations, and ROps (ROPs).
• Strong coordination, organization, milestone tracking, prioritization, and follow-through skills.
• Experience with audits, self-assessments, gap analysis, evidence management, corrective action tracking, and mock tracer activities.

THE ROLE

The Accreditation Project Manager leads organization-wide preparation for the Accreditation Canada survey across peopleCare Communities' long-term care homes. Reporting to the VP Quality, Research, and Strategic Partners the role coordinates work plans, evidence, gap closure, staff readiness, and survey activities in partnership with domain leads and home leadership.

This position combines project coordination, document and evidence management, audits, education, and on-site support to promote consistent practice and survey readiness across the organization. The goal is to support Full corporate Accreditation Status during the May 2027 survey.

What You’ll Be Responsible For

• Coordinate the organization-wide accreditation readiness plan, milestones, deliverables, meetings, and status tracking through the May 2027 survey.
• Work with the VP Quality, Research, and Strategic Partners Domain Team Leads, Home Leadership Teams, Community Partners, and Resident and Family Councils to align priorities and follow up on assigned actions.
• Standardize SharePoint and Accreditation Canada portal structures, evidence folders, templates, naming conventions, and proof-of-compliance records across homes.
• Review historical survey results, satisfaction data, quality indicators, and known risks; facilitate self‑assessments, gap analysis, and corrective action planning.
• Coordinate Required Organizational Practice (ROP) mapping, action plans, and supporting evidence across key safety domains, including infection prevention and control, medication safety, clinical care, and leadership/safety culture.
• Conduct or coordinate site walkthroughs, chart and staff file reviews, environmental audits, policy checks, and other compliance reviews; document gaps and track corrective actions to completion.
• Maintain the accreditation document matrix and ensure required policies, plans, program reviews, training records, quality/risk evidence, and life‑safety documentation are current and survey‑ready.
• Develop and deliver accreditation communications, education, and survey‑preparation sessions for leaders, frontline teams, residents, families, and councils.
• Coordinate stakeholder interview and focus‑group readiness, including Community Partner and Client/Family Engagement groups, and support executive survey preparation materials.
• Plan and conduct mock tracer exercises to validate staff knowledge, physical readiness, documentation, and confidence before the survey.
• Provide on-site survey‑week support and coordinate real‑time communication among homes, leadership teams, and surveyors during interviews, facility reviews, document audits, and debriefs.
• Maintain shared schedules, tracking tools, and confidential project information in accordance with applicable privacy requirements and internal data‑security practices.’
• Other duties as assigned.

What You Bring

• Degree in health care, quality improvement, health administration, project management, or a related field; equivalent relevant experience may be considered.
• Minimum 5 years of experience in a leadership and or project management role in a healthcare setting.
• Proven experience supporting accreditation, quality improvement, compliance, clinical governance, or complex health‑care projects; long‑term care or multi‑site experience is an asset.
• Working knowledge of Accreditation Canada standards, survey processes, evidence expectations, and Required Organizational Practices (ROPs).
• Strong project coordination, organization, milestone tracking, prioritization, and follow through skills.
• Experience with audits, self‑assessments, gap analysis, evidence management, corrective action tracking, and mock tracer activities.
• Strong SharePoint and Microsoft Office skills, with confidence using digital portals, trackers, and document repositories.
• Excellent communication, facilitation, education, presentation, and stakeholder engagement skills.
• Knowledge of long‑term care quality and safety practices, including infection prevention and control, medication safety, emergency preparedness, workplace violence prevention, and resident safety.\
