TheOutreach Support Mentor is responsible for providing direct support to theparticipants of the Outreach or Westman Services program ensuring allprogram requirements are consistently being met. The Outreach Support Mentorwill work alongside the Drop in Coordinator and Clinical team. This includes participating in theongoing planning, monitoring, and communication of activities of the programparticipants that are in relation to each participant’s program planningdocuments. The Outreach Support Mentor is expected to follow the PersonnelPolicies, including Workplace Health and Safety Policies, the OperationalPractices and the goals of Life’s Journey Inc. in completing their duties. TheOutreach Support Mentor is also responsible for limited program administration.
Thisposition works collaboratively with other members of the Outreach Team including other Outreach Support Mentors, CaseWorkers, Clinical Case Managers, Assistant Coordinator, Outreach Coordinatorand Program Manager. In addition to the Outreach Team, the Outreach Support Mentor is also responsible to ensure positiveinteractions with collaterals associated with the participants being supported.
