Job summary

Life’s Journey Inc. in Brandon, MB seeks an Outreach Support Mentor (Full Time permanent) to provide direct support to participants of the Westman Services program.

The role works Tue-Sat 9:30am-5:00pm with a wage of $20.29–$23.38 per hour, contingent on qualifications and program needs. The successful candidate will collaborate with the Clinical team and Case Workers, ensuring program planning documents are followed, and will participate in ongoing planning, monitoring and communication of

Qualifications

• Previous Outreach experience required.
• Prefer two years in a related role serving individuals with disabilities.
• Experience in an outreach setting.
• Satisfactory checks: Child and Adult Abuse Registry, Criminal Record Check (vulnerable sector), drivers' abstract and license.
• First Aid Level 1 – Emergency certification.
• Reliable vehicle and Class 5 or equivalent driver's license.
• Ability to attend work as scheduled.
• Strong written and verbal communication.
• Demonstrated cultural competence and willingness to participate in agency training.
• Safe and reliable vehicle with appropriate insurance.

Responsibilities

• Provide direct support to participants of the Outreach/Westman Services program, ensuring program requirements are met.
• Collaborate with Drop in Coordinator and Clinical team; participate in ongoing planning, monitoring, and communication of participant activities.
• Follow Personnel Policies and Workplace Health and Safety policies; perform limited program administration.

OVERALL RESPONSIBILITY

TheOutreach Support Mentor is responsible for providing direct support to theparticipants of the Outreach or Westman Services program ensuring allprogram requirements are consistently being met. The Outreach Support Mentorwill work alongside the Drop in Coordinator and Clinical team. This includes participating in theongoing planning, monitoring, and communication of activities of the programparticipants that are in relation to each participant’s program planningdocuments. The Outreach Support Mentor is expected to follow the PersonnelPolicies, including Workplace Health and Safety Policies, the OperationalPractices and the goals of Life’s Journey Inc. in completing their duties. TheOutreach Support Mentor is also responsible for limited program administration.

Thisposition works collaboratively with other members of the Outreach Team including other Outreach Support Mentors, CaseWorkers, Clinical Case Managers, Assistant Coordinator, Outreach Coordinatorand Program Manager. In addition to the Outreach Team, the Outreach Support Mentor is also responsible to ensure positiveinteractions with collaterals associated with the participants being supported.

QUALIFICATIONS & EXPERIENCE

• Previous Outreach experience Required


• Optimally, two years’ experience in a related role and supporting individualswith disabilities.


• Experience in an outreach setting.


• Satisfactory Child and Adult Abuse Registry check, satisfactory Criminal RecordCheck (vulnerable sector search) and a satisfactory Drivers’ abstract andLicense.


• Certification of First Aid Level 1 – Emergency.


• Reliable vehicle


• Possess a 5F Driver’s License


• The ability to attend work regularly as scheduled or requested.


• Demonstrated language, writing, comprehension skills.


• Demonstrate cultural knowledge competence and participate in agency trainingfor development.
