Job Purpose:

Housing First is a program of the North End Community Health Centre in Halifax. This program is funded both by the federal government under the Homelessness Partnering Strategy and the provincial government. All activities associated with Housing First will be managed by the manager of Housing First.

Education and Experience:
  • Human services and/or relevant post-secondary education (ie-psychology, social work) in a related field is required and/or 2 years equivalent work experience.
  • Experience working with people with lived experience or are at risk.
  • 5+ years of experience working with participants experiencing homelessness.
Position Responsibilities.:
  • Conduct the tenant selection process through the BNL list in collaboration with internal teams and community partners.
  • Responsible for facilitating monthly tenant meetings and programming.
  • Provide crisis response and ongoing participant support in collaboration with the property management team.
  • Develop, implement, and monitor strength-based recovery plans grounded in Housing First and Harm Reduction principles.
  • Manage a caseload of 15–30 participants, providing comprehensive and temporary case management support as required.
  • Deliver support services within both Place-Based and Scattered Site housing programs to promote housing stability and participant well-being.
  • Assist participants in securing housing by facilitating housing searches, property viewings, rental applications, and move-in coordination.
  • Provide accompaniment with participants to appointments and community services as needed
  • Maintain accurate, timely, and confidential documentation in accordance with Housing First standards and organizational policies.
  • Advocate on behalf of participants and connect them to appropriate community resources, programs, and support services.
  • Foster effective communication and collaborative relationships with service providers, landlords, community agencies, and stakeholders.
  • Support community-building initiatives, facilitate participant engagement, and actively participate in team meetings while maintaining confidentiality.
  • Represent the organization and program in a professional manner while adhering to Housing First practices and organizational values.
  • Collaborate with internal departments, external agencies, and community partners to ensure coordinated and effective service delivery.
  • Contribute to organizational development initiatives and participate in continuous program improvement activities.
  • Maintain consistent communication with the Manager and team members while demonstrating flexibility in adapting to operational needs.
  • Facilitate monthly house meetings in partnership with property management and other relevant stakeholders.
  • Assemble furniture and prepare units to ensure accommodations are safe, functional, and move-in ready for tenants.
  • Support tenants through move-out processes and coordinate storage arrangements when applicable.
  • Provide health-focused case management support directly and/or in collaboration with healthcare professionals and community practitioners.
  • Build and maintain collaborative relationships with external stakeholders and community partners to support participant outcomes.
  • Coordinate access to essential supports and services, including legal aid, immigration resources, housing retention programs, and other community-based services.
  • Perform additional duties and responsibilities as assigned to support program operations and organizational objectives
