Transition Age Youth Case Manager: TIP Program (LV)

Join an expanding team to support young adults with mental health diagnoses. Access Services is hiring a Transition Age Youth Case Manager for the Transition to Independence Process (TIP) program serving Lehigh and Northampton Counties, based in Bethlehem, PA.

What You'll Do

As a Transition Age Youth Case Manager, you will work with young adults aged 16-26, using the evidence-based TIP Model to provide individualized, strengths-based planning. You will engage in one-on-one and group settings across community locations, managing a caseload of 15 young adults. Your role involves building relationships, guiding access to resources, and fostering resilience to support their transition to adulthood.

We seek a high-character, team-oriented individual passionate about mental health and social services, with a strength-focused, non-judgmental approach.

Work Schedule

Full-time, 40 hours/week, Monday-Friday, with flexibility for occasional adjustments. Some evening and weekend hours are required for events.

Requirements
  • Passion for community service, experience engaging diverse populations, familiarity with community resources
  • Experience in mental health or with transition age youth
  • Computer skills
  • Excellent communication skills
  • Valid driver's license, 2+ years driving experience, reliable vehicle, insurance
Education
  • Bachelor's degree with at least 12 college credits in Human Services, Psychology, Social Work, Sociology, or related field

OR

  • High School Diploma + 12 semester credits in social sciences + 2 years direct care experience with mental health clients

Benefits

Pay: $20.00-20.50/hr with performance-based increases and incentives. On-call stipend, provided laptop and cell, mileage reimbursement.
