UnitingCare Queensland is seeking a Supported Living Facilitator to join our FaDS team in Slacks Creek. This casual role focuses on assisting people with daily living, personal care and supporting their autonomy and life goals within a respectful, safe environment.

You will collaborate with families and networks to implement individual plans, uphold rights, and promote quality of life for people with disabilities, while working flexibly to cover 24-hour rostering needs.

• Remuneration: $45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

• Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives
• Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life
• Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people
• Enable people to attain personal autonomy over their own lives and the services which they receive
• Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family

What makes a difference for us?

• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Great availability across 24 hour SIL rostering (Sleepovers, Morning, Afternoon and Evening shifts)
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn
• A current NDIS Worker Screening check
• Open Australian Drivers License
• Current CPR and First Aid Certificate (required before you start)
