A stealth-mode healthcare AI startup is looking for a Principal AI Engineer to build and refine innovative AI solutions for healthcare revenue management. This fully remote position offers a competitive salary and significant impact on the product direction. The ideal candidate has extensive programming skills, experience with large language models, and a meticulous approach to product development.
WHO WE ARE
We are a stealth-stage healthcare AI startup that is the missing link in healthcare revenue management, bridging the gap between clinical care and financial operations. We're transforming how primary care clinics capture revenue through innovative AI technology that operates at the intersection of medical documentation, billing, and value-based care compliance.
In an industry where medical providers leave approximately 16% of their revenue uncollected due to insurance denials and compliance gaps, our mission is clear: enable healthcare providers to capture every dollar they've earned while uncovering new revenue opportunities.
Founded by experienced entrepreneurs with successful exits in healthcare technology and backed by advisors from leading health insurance companies, we are building a virtuous cycle that improves both patient care and provider profitability. We've already secured our first client contract and are positioned for a major pilot.
Our technology tackles critical challenges facing healthcare providers:
Join us as we build technology that enables healthcare providers to thrive in an era of value-based care, maximizing revenues in primary care at scale.
WHY THIS ROLE IS PERFECT FOR YOU
This is your opportunity to shape production LLM applications that directly impact healthcare revenue and patient care. You'll work on a recently deployed AI system that automates medical coding and billing, evolving it from good to exceptional through meticulous refinement, rigorous testing, and continuous improvement.
You're not just implementing AI solutions—you're building a system that must get medical coding right the first time, understand complex insurance rules that change constantly, learn from denials, and continuously improve. Every line of code you write directly affects whether healthcare providers get paid and patients receive proper care.
KEY RESPONSIBILITIES
Very hands-on—we're looking for a builder who obsesses over details!
1. Build and Refine AI Products
Develop and implement new business rules for medical consultation billing codes, adapting to changes from various insurance providers
Propose and implement enhancements to our existing LLM-based products and optimize prompt engineering for maximum accuracy
Use AI-assisted coding tools like Cursor as part of your daily workflow to rapidly build and iterate on solutions
2. Ensure Quality and Accuracy
Conduct extensive testing in staging environments and perform detailed debugging to ensure business rules are accurately implemented—in healthcare, "mostly right" isn't good enough
Verify data consistency from input to output to guarantee the solidity of our AI products
Monitor denial patterns and implement feedback loops that continuously improve model accuracy
3. Own the Development Lifecycle
Be responsible for the entire process from understanding business rules to deploying and validating code in production
Debug complex system integrations and validate AI outputs against medical coding standards
4. Collaborate and Drive Improvement
Work closely with AI, Tech, and Data Engineering teams to maintain complete understanding of the end-to-end process
Proactively identify critical issues, propose improvements, and stay current with new tools and techniques in the AI and LLM space
In the near future, contribute to fine-tuning specialized LLM models with our internal knowledge base
REQUIRED EXPERTISE
1. Technical Skills
Strong proficiency in programming languages like Python or Go
Hands-on experience building products on top of large language models (LLMs)
Experience with LLM APIs (AWS Bedrock, OpenAI, Anthropic Claude, or similar)
Strong prompt engineering skills with focus on accuracy and consistency
Solid understanding of how LLMs work and how to leverage them for production applications
Comfortable working with cloud platforms (AWS preferred) and databases (PostgreSQL)
2. Mindset & Approach
Meticulous attention to detail with a strong capacity for debugging and code validation
Ability to work effectively in a fast-paced, ever-changing startup environment
Proven ability to learn new tools, frameworks, and business rules quickly
A self-starter who can work independently but also collaborate effectively within a small team
Possesses a "big picture" perspective, understanding how your work fits into the entire end-to-end process from client data entry to insurance billing
3. Highly Valued Qualifications
Experience with AI-assisted coding tools like Cursor, GitHub Copilot, or similar
Experience working in a startup or similar environment with constant change
Familiarity with RAG (Retrieval Augmented Generation) architectures
Experience with LLM orchestration frameworks (LangChain, LlamaIndex, or similar)
Previous experience in healthcare technology, particularly with medical coding, billing, or revenue cycle management
Healthcare Domain Knowledge: Not required, but you must be willing and able to learn medical coding, billing rules, and insurance requirements quickly. We'll teach you what you need to know.
WHAT WE OFFER
1. Unparalleled Growth Opportunity
Ground-floor opportunity in a stealth-mode company with massive market potential
Direct impact on product direction and technical architecture decisions
Opportunity to build and lead a team as we scale
2. Professional Excellence
Work alongside world-class engineers and healthcare industry veterans
Access to cutting-edge LLM technologies solving complex, real-world problems
Opportunity to influence architectural decisions as we scale our AI infrastructure
Competitive compensation package with significant equity upside
3. Work-Life Integration
Fully remote position with flexible hours accommodating Brazil time zones
Fast-paced environment where you'll learn constantly and be challenged daily
Opportunity to work with prestigious American healthcare clients
Meaningful work that directly improves healthcare outcomes and provider sustainability
READY FOR THE CHALLENGE?
If you're a principal-level engineer who loves production AI systems, thrives on continuous improvement, obsesses over getting details right, and wants to make a real impact in healthcare, this role offers the perfect combination of technical challenge, business impact, and startup energy.
Apply now and help us transform healthcare revenue management through the power of data and AI.
