As a member of the Home and Community Care team, you will provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks and coordinates follow-up care to improve health and well-being.

Note: Employees must be in active regular status and remain in role for a minimum of 12 months from hire/rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of student loan repayments.
Salary and Benefits: The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on factors including local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws.
