As a member of the Home and Community Care team, you will provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks and coordinates follow-up care to improve health and well-being.

Primary Responsibilities
  • Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings including point-of-care testing (as appropriate)
  • Identify diagnoses for care management and active medical management to support treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings
  • Communicate assessment findings to inform the member’s PCP of potential gaps in care
  • Identify urgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease processes, medications, and compliance

Required Qualifications
  • Active NP or PA license or the ability to obtain by start date. Licensure must be unencumbered, free of open/unresolved disciplinary actions including probation or restrictions against privilege to practice
  • For NPs – Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care; or for PAs – Physician Assistant national certification through NCCPA
  • Ability to complete physical requirements, including lifting a 30-pound bag in/out of a car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessments
  • Reliable transportation to complete home visit assessments
Preferred Qualifications
  • 1+ years of clinical experience in the highest level of education, clinical setting
  • May be requested to obtain additional licensure in other geographic areas
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
  • Ability to travel throughout the state up to 5% of the time; eligible for additional compensation incentives

Note: Employees must be in active regular status and remain in role for a minimum of 12 months from hire/rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of student loan repayments.

Salary and Benefits: The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on factors including local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws. Benefits include a comprehensive package, incentive and recognition programs, equity stock purchase and 401k contribution, with eligibility requirements.
