Job Description

An experienced RN responsible for the overall management of patients, including direct care provision with a designated visit productivity standard. This position involves planning, organizing, and directing all patient care services for patients in the assigned caseload, ensuring compliance with current standards and regulations to maintain high-quality care. The RN coordinates with the interdisciplinary team to implement the established patient care plan and manages visit utilization and expenses. Additionally, the RN may assist in agency-wide orientation, competency evaluations, and provide consultation to clinical staff based on specific expertise.

Qualifications

Education:

• Graduate of an accredited School of Nursing

Certification & Licensure:

• RN-Registered Nurse of California

Experience:

• Minimum 2 years of recent relevant experience

Skills and Knowledge

• Ability to function independently and as part of an interdisciplinary team; exercise sound nursing judgment.
• Knowledge of pathophysiology and treatment protocols for common home care diagnoses.
• Understanding of safety and infection control principles.
• Strong clinical assessment skills.
• Ability to recognize the needs of hospice and home care patients.
• Effective communication skills with patients and families.
• Ability to maintain professional relationships with internal and external stakeholders.
• Understanding of palliative care for terminally ill patients.
