Job Description

• Care coordinating caseload of EIP clients, supporting with EIP duty and medication administration
• A Qualified practitioner with ability to manage caseload within community setting with support and supervision. Assessment of new referrals to service. Liaising with clients, carers and families via telephone, video conferencing or face-to-face. Covering Duty. Taking part in regular MDTs.

Requirements

• Experience working in the community with mental health clients
• Experience of working within a community older adult or Memory Service team. Qualified mental health practitioner who has knowledge of working with and managing a caseload autonomously in the community.
