Job summary

AnglicareSA in Adelaide is seeking a Support Facilitator to work with young people with lived care experience, guiding them toward independent living through strengths-based case management and coordinated supports.

You'll collaborate with families and service providers to access housing, education, employment and community resources, building confidence and financial literacy while centering the voices of young people.

Qualifications

• Must hold a Bachelor’s Degree in Social Work (AQF Level 7).
• Experience working with Aboriginal and Torres Strait Islander families and communities.
• Experience delivering trauma-informed practice.

Responsibilities

• Delivering trauma-informed, strengths-based case management and support coordination, including screening, risk assessment, goal setting and client-led planning.
• Supporting young people to develop independent living skills, navigate housing and tenancy options, build financial literacy and access education, training and employment opportunities.
• Coordinating referrals, advocating for young people and connecting them with services, natural supports and community networks that strengthen their wellbeing and independence.
• Facilitating collaborative, client-led decision-making and supporting young people to access relevant grants, allowances and practical resources as they transition from care.
• Building safe, culturally responsive relationships and working collaboratively with families and service providers to ensure young people’s needs, cultural identities and goals remain central to their support.
