Sydney Children’s Hospitals Network in Westmead is seeking a Youth Peer Support Worker to assist mental health consumers transition from acute inpatient to community care. You will work within a multidisciplinary team, using lived experience to engage young people and support recovery goals.

This non-clinical role applies the CHIME framework (Connectedness, Hope, Identity, Meaning, Empowerment) and involves advocacy, collaboration with clinical staff, and community follow-up to improve outcomes

Responsabilités

• Assist mental health consumers' transition from Acute Inpatient to Community Care within a multidisciplinary team.
• Provide non-clinical support drawing on the CHIME framework (Connectedness, Hope and optimism, Identity, Meaning, Empowerment).
• Engage with consumers and clinical teams to implement recovery goals and provide assertive community follow-up.
• Advocate for recovery-focused care and contribute to development of models of care and service delivery.
