Job summary

UnitingSA is seeking a dedicated Tailored Learning Case Manager to support young people aged 12–25 at risk of disengaging from education. Based in Noarlunga Centre, you will deliver strengths-based case management, advocacy and mentoring to help participants reconnect with education, training, employment and community opportunities.

You will collaborate with schools and community partners, maintain records and contribute to service improvement, while meeting funding requirements and promoting

Qualifications

• Degree in Social Work or equivalent preferred.
• Current South Australian Driver's Licence.
• Current Working with Children Check.
• Current NDIS Worker Screening Check.
• Safe Environments for Children and Young People certificate.
• RRHAN-EC certification.

Responsibilities

• Provide strengths-based case management and support to young people facing barriers to education, training, and employment.
• Assess needs, develop individual plans, coordinate referrals, and advocate for positive outcomes.
• Deliver engaging programs and activities that build resilience, life skills, confidence, and pathways to learning or employment.
• Build and maintain relationships with schools, community organisations, referral agencies, and government stakeholders.
• Support young people to achieve their goals through collaboration, community engagement, and access to services.
• Maintain accurate records, reporting, and compliance with funding and program requirements.
• Contribute to program development, evaluation, and continuous improvement.
