Job summary

Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults aged 9-25 as they navigate life with a disability. You will lead an interdisciplinary youth team and be the main contact for participants and families.

You will work across homes, schools, and community settings, using your assessment and intervention skills to help participants reach educational, personal, social, and working life goals while documenting and billing sessions through our system.

Qualifications

Must have a relevant tertiary qualification and registration.
Youth Worker: Bachelor/Master’s in Youth Work and YWA membership.
Social Worker: Bachelor/Master’s in Social Work and AASW membership.

Responsibilities

Lead the interdisciplinary youth team planning an NDIS funded therapy package, acting as main contact for adolescents and young adults and their families.
Use assessment and intervention skills to help participants achieve educational, personal, social, and work-life goals.
Work across diverse settings (homes, schools, playgrounds, workplaces, community centres) within the social model.
Engage in future-focused conversations with participants and their supports.
Document and bill for each session via the online participant management system.
Attend weekly team meetings and collaborate with Team Leader and Therapy Mentor.
