Job summary

Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults (9-25) with disability, across Melton and nearby regions. You’ll lead a Youth Team, coordinate NDIS-funded therapy packages, and work across homes, schools, and community settings to help participants achieve education, personal, social, and work-life goals.

Role requires a relevant qualification with registration, plus two years’ experience with youth and a willingness to work toward billable

Qualifications

• Relevant tertiary qualification and registration required.
• Youth Worker option: Bachelor or Master’s in Youth Work with YWA membership.
• Social Worker option: Bachelor or Master’s in Social Work with AASW membership.

Responsibilities

• Lead the interdisciplinary youth team planning an NDIS funded therapy package.
• Support youth participants to achieve educational, personal, social, and working-life goals.
• Deliver therapy across diverse settings including homes, schools and community spaces.
• Document sessions and coordinate with participants’ supports.
