Job summary

UnitingCare Queensland is seeking a Case Manager within the Aspire program to provide trauma-informed, strengths-based support to vulnerable young people. You’ll conduct intake, assessment and planning, and work with families, schools and services to coordinate care.

This role supports housing, education, training, employment and wellbeing, developing integrated service responses with internal and external partners while keeping accurate client records and contributing to a positive team culture.

Qualifications

• A minimum undergraduate qualification in Social Work, Psychology, Counselling, Human Services or related field.
• Experience working with vulnerable children and young people in case management or therapeutic support.
• Knowledge of child/adolescent development and effects of trauma.
• Ability to build trusting relationships with young people and engage goal-focused plans.
• Experience collaborating with families, government agencies and community stakeholders.
• Strong assessment, problem-solving, advocacy and coordination skills.
• Excellent written and verbal communication with accurate documentation.
• Commitment to culturally safe practice with Aboriginal and Torres Strait Islander peoples.

Responsibilities

• Conducting intake, assessment and individual support planning.
• Providing case management, therapeutic support and advocacy to young people.
• Supporting young people to access housing, education, training, employment and community services.
• Coordinating integrated responses and maintaining effective partnerships with internal and external stakeholders.
• Facilitating and evaluating group programs and activities that support youth development and wellbeing.
• Maintaining accurate and timely client records, assessments, case plans, and reports.
• Monitoring client progress and outcomes to inform intervention planning and service delivery.
• Contributing to a positive team culture through reflective practice, collaboration and continuous improvement activities.
