Job summary

UnitingCare Queensland is seeking a Case Manager to join the Aspire Program in Townsville. You will provide trauma-informed, strengths-based support to vulnerable young people, through assessment, planning, advocacy and coordinated service responses.

Regular travel between Townsville and Ayr is required, with a focus on housing, education, employment and wellbeing outcomes. You will work with families, schools and government agencies to build resilience and sustainable support, contributing to

Qualifications

• Undergraduate qualification in Social Work, Psychology, Counselling, Human Services or related behavioural science.
• Experience with vulnerable children and young people in case management or therapeutic support.
• Current Queensland Driver Licence and Blue Card.

Responsibilities

• Conduct intake, assessment and individual support planning.
• Provide case management, therapeutic support and advocacy to young people.
• Coordinate access to housing, education, training, employment and community services.
• Maintain client records, reports and case plans.
• Collaborate with families, schools, agencies and community stakeholders to achieve outcomes.
• Monitor progress and contribute to program development and continuous improvement.
