Job summary

UnitingCare Queensland is seeking a Case Manager within the Aspire program to provide trauma-informed, strengths-based support to vulnerable young people. You’ll conduct intake, assessment and planning, and work with families, schools and services to coordinate care.

This role supports housing, education, training, employment and wellbeing, developing integrated service responses with internal and external partners while keeping accurate client records and contributing to a positive team culture.

Qualifications

  • A minimum undergraduate qualification in Social Work, Psychology, Counselling, Human Services or related field.
  • Experience working with vulnerable children and young people in case management or therapeutic support.
  • Knowledge of child/adolescent development and effects of trauma.
  • Ability to build trusting relationships with young people and engage goal-focused plans.
  • Experience collaborating with families, government agencies and community stakeholders.
  • Strong assessment, problem-solving, advocacy and coordination skills.
  • Excellent written and verbal communication with accurate documentation.
  • Commitment to culturally safe practice with Aboriginal and Torres Strait Islander peoples.

Job description

Employment Term: Maximum Term to 30 June 2027, Full-Time (38 hours per week)

Location: Burdekin/Townsville - This role requires regular travel between Townsville and Ayr.

Remuneration: $53.43p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

The Aspire program supports vulnerable young people who may be experiencing family conflict, homelessness, disengagement from education, mental health challenges, social isolation or other complex life circumstances. Aspire works alongside young people to build resilience, strengthen connections and support positive outcomes across housing, education, employment, wellbeing and community participation.

As a Case Manager with Aspire, you will provide trauma-informed, strengths-based support to young people through assessment, case planning, therapeutic interventions, advocacy and coordinated service responses. You will work collaboratively with young people, families, schools, Child Safety, Youth Justice, health services and other community stakeholders to support sustainable outcomes and help young people achieve their goals.

The key responsibilities include, but are not limited to:

Conducting intake, assessment and individual support planning.

Providing case management, therapeutic support and advocacy to young people.

Supporting young people to access housing, education, training, employment and community services.

Coordinating integrated responses and maintaining effective partnerships with internal and external stakeholders.

Facilitating and evaluating group programs and activities that support youth development and wellbeing.

Maintaining accurate and timely client records, assessments, case plans, and reports.

Monitoring client progress and outcomes to inform intervention planning and service delivery.

Contributing to a positive team culture through reflective practice, collaboration and continuous improvement activities.

What makes a difference for us?

A minimum undergraduate qualification in Social Work, Psychology, Counselling, Human Services or a related behavioural science discipline.

Demonstrated experience working with vulnerable children and young people within a case management, youth work or therapeutic support framework.

Knowledge of child and adolescent development, including the impacts of trauma, family conflict, violence, grief and loss.

Ability to build trusting relationships with young people and engage them in goal-focused support planning.

Demonstrated experience working collaboratively with families, government agencies and community stakeholders to achieve positive outcomes.

Strong assessment, problem-solving, advocacy and case coordination skills.

High-level written and verbal communication skills, including the ability to maintain accurate client documentation and case records.

Demonstrated commitment to culturally safe practice and working effectively with Aboriginal and Torres Strait Islander young people and young people from diverse backgrounds.

Current Queensland Driver Licence.
