Job summary

Ballarat Community Health, as the lead agency for headspace Ballarat, is seeking a Youth Worker to support young people aged 12-25 through intake, psychosocial assessment and short-term interventions. You will help young people feel welcome and connected to the services they need, working with families, schools and community services.

The role focuses on intake, assessment, brief interventions, and maintaining accurate case notes.

Qualifications

• Hold a tertiary qualification in Youth Work, Social Welfare, Social Work or a related field.
• Experience supporting young people in community, health or social service settings.

Responsibilities

• Manage intake and psychosocial assessment for young people aged 12–25.
• Provide brief interventions or short-term support to young people.
• Connect young people with services that meet their needs.
• Maintain accurate case notes and documentation.
