Job summary

BlueCare Mackay Homefield Aged Care Facility is seeking a Wellness Partner to deliver person-centred wellness programs for residents. You will facilitate group and individual activities, support independence and wellbeing, and work with the Wellness Lead, care teams, families and volunteers to create meaningful participation.

Requirements include a Certificate IV in Leisure and Health, a current QLD driver license, and experience in aged care or related settings.

Qualifications

• Certificate IV in Leisure and Health or equivalent qualification.
• Current Queensland Driver Licence.
• Experience facilitating group and individual activities in aged care or related settings.
• Knowledge of person-centred practice, dementia care and cultural safety.

Responsibilities

• Facilitate group and one-on-one wellness activities tailored to residents.
• Build positive relationships with residents through meaningful engagement.
• Support residents' independence, choice and wellbeing.
• Collaborate with the Wellness Lead, care teams, families and volunteers to deliver programs.
• Assist with wellness assessments, documentation and reporting changes in resident wellbeing.

About therole:

As a Wellness Partner, you'lldeliver engaging, person-centredwellness programs that support residents' physical, emotional,socialand cognitive wellbeing. Working closely with residents,familiesand the wider care team, you'llhelp create opportunities for meaningful participation,independenceand connection.

You will:

• Facilitate group and one-on-one wellness activities tailored to residents' interests and abilities
• Build positive relationships with residents through meaningful engagement and companionship
• Support residents' independence, choice,dignityand wellbeing
• Collaborate with the Wellness Lead, care teams,familiesand volunteers to deliver engaging programs
• Assistwith wellness assessments,documentationand reporting changes in resident wellbeing
• Support volunteers to contribute confidently to resident activities and experiences
• Prepare andmaintainsafe, welcoming activity spaces and equipment
• Followorganisationalpolicies and contribute to continuous improvement initiatives

Role Requirements:

• Certificate IV in Leisure and Health or equivalent qualification
• Current Queensland Driver Licence
• Experiencefacilitatinggroup and individual activities in aged care,communityor disability settings
• Understanding of person-centred practice, dementiacareand cultural safety
• Ability to adapt activities to support a range of physical and cognitive abilities
• Experience working collaboratively within a multidisciplinary team
• Knowledge of infection control, manualhandlingand aged care quality standards
• Strong communicationand relationship-building skills
• Confidence to satisfy the relevant probity checks required by legislation or organisational policies
