Job summary

BlueCare Mackay Homefield is seeking a dedicated Wellness Partner to deliver engaging, person-centred wellness programs for residents in our aged care facility. You will work closely with residents, families and the care team to create meaningful activities that promote physical, emotional, social and cognitive wellbeing.

You will facilitate group and one-on-one activities, support independence and dignity, and help with wellbeing assessments and reporting.

Qualifications

• Certificate IV in Leisure and Health or equivalent qualification.
• Current Queensland Driver Licence.
• Experience facilitating group and individual activities in aged care, community, or disability settings.
• Understanding of person-centred practice, dementia care and cultural safety.

Responsibilities

• Facilitate group and one-on-one wellness activities tailored to residents' interests.
• Build positive relationships with residents through engagement and companionship.
• Support residents' independence, choice, dignity and wellbeing.
• Collaborate with Wellness Lead, care teams, families and volunteers.
• Assist with wellness assessments, documentation and reporting changes.
• Prepare and maintain safe, welcoming activity spaces and equipment.
• Follow policies and contribute to continuous improvement initiatives.
