Job summary

BlueCare Mackay Homefield Aged Care Facility is seeking a friendly Wellness Partner to deliver engaging, person-centred activities that support residents' physical, emotional, social and cognitive wellbeing. You will work with residents, families and the care team to create meaningful participation, support independence and dignity, and help track wellbeing.

This permanent full-time role offers a supportive team, salary packaging and Not-for-Profit benefits.

Qualifications

• Certificate IV in Leisure and Health or equivalent qualification.
• Current Queensland Driver Licence.
• Experience facilitating group and individual activities in aged care, community or disability settings.
• Understanding of person-centred practice, dementia care and cultural safety.
• Ability to adapt activities to support a range of physical and cognitive abilities.
• Experience working collaboratively within a multidisciplinary team.
• Knowledge of infection control, manual handling and aged care quality standards.
• Strong communication and relationship-building skills.
• Confidence to satisfy probity checks required by legislation or organisational policies.

Responsibilities

• Facilitate group and one-on-one wellness activities tailored to residents' interests and abilities.
• Build positive relationships with residents through meaningful engagement and companionship.
• Support residents' independence, choice, dignity and wellbeing.
• Collaborate with the Wellness Lead, care teams, families and volunteers to deliver engaging programs.
• Assist with wellness assessments, documentation and reporting changes in resident wellbeing.
• Prepare and maintain safe, welcoming activity spaces and equipment.
• Follow organisational policies and contribute to continuous improvement initiatives.
