Job summary

Anglicare NT in Darwin is seeking a ReConnect Youth & Family Worker to provide youth-centred, family-focused case management and practical support to vulnerable young people and their families.

Working within a strengths-based, trauma-informed framework, you will deliver case management, perform assessments, develop plans, facilitate group programs and collaborate with government and community services. This is a part-time fixed-term position until 30 June 2027.

Qualifications

• Qualifications in Human Services, Social Sciences or a related field.
• Experience working with vulnerable or high-risk young people and families.
• Knowledge of youth, family support, homelessness and child protection systems.
• Experience applying trauma-informed, strengths-based and youth-focused practice frameworks.
• Strong assessment, case management and engagement skills.
• Experience facilitating groups and workshops for young people.
• Excellent communication, relationship-building and stakeholder engagement skills.

Responsibilities

• Deliver case management and support services to young people and families.
• Conduct assessments and develop individualised case plans.
• Facilitate group programs that build life skills, social connection and wellbeing.
• Use assertive outreach strategies to engage young people experiencing complex challenges.
• Work collaboratively with government, community and family support services.
• Support community capacity-building initiatives and awareness activities.
• Maintain high-quality client records, data and reports.
