UnitingCare Children, Young People and Families – Toowoomba QLD is seeking an Outreach Support Worker for a permanent part-time role (32 hours per week) in Toowoomba. You’ll provide trauma-informed, CARE-based support to children and young people up to 18 with moderate to extreme needs, promoting safety and wellbeing.

You’ll conduct needs assessments, plan supports, help youths build life skills, and collaborate with family and community services to achieve positive outcomes.

Qualifications

• Minimum Certificate IV in Child, Youth & Family Intervention or related field.
• Experience applying CARE principles when supporting children and young people.
• Willingness to undergo a personal history check under the Child Protection Act 1999 (LCS2).
• Strong understanding of the child protection system and exposure to out-of-home care environments.
• Current Working with Children Check (Blue Card), a valid Queensland Driver Licence, and current First Aid & CPR.

Responsabilités

• Conduct initial needs assessments to identify strengths and set goals for placement stability.
• Undertake case planning and coordinate supports to meet identified goals.
• Work with children and young people to develop life skills and improve wellbeing.
• Build positive relationships with service providers, government departments and community agencies.
• Maintain accurate quality records in line with reporting requirements and organizational systems.
• Maintain professional skills through training, supervision and reflective practice.
