UnitingCare Queensland is seeking an Outreach Support Worker to join the FaDS team in Toowoomba. This permanent part-time role (32 hours per week) offers $46.70 per hour plus super and Not-for-Profit salary packaging.

You will support children and young people with moderate to extreme needs using CARE principles in a trauma-informed framework. You'll conduct initial assessments, plan supports, foster wellbeing, and collaborate with families and service providers while maintaining accurate

Formación

• A minimum of a Certificate IV in Child, Youth & Family Intervention or Cert IV in Human Services.
• Experience incorporating CARE Principles when supporting Children and Young People.
• Willingness to undergo a personal history check under the Child Protection Act 1999 (LCS2).
• Strong understanding of the child protection system and exposure to residential or out-of-home care.

Responsabilidades

• Conduct initial needs assessments to identify strengths and develop goals.
• Undertake case planning and manage supports to meet case goals.
• Work with children and young people to develop daily living skills and wellbeing.
• Develop and maintain relationships with service providers and community agencies.
• Maintain accurate quality records and reporting.
• Engage in ongoing professional development and reflective practice.
