Job summary

St Vincent’s Health Australia in Sydney seeks a Transplant Care Coordinator to join the Heart & Lung Clinic on a max-term, part-time basis (8 hours/week) with on-call duties. The role covers patients through heart and/or lung transplant pathways across inpatient and outpatient settings.

Qualifications

• Currently registered as a Registered Nurse (Division 1) with AHPRA.
• Relevant post-registration qualifications and at least three years’ experience in the clinical area of their post-graduate qualification.
• Demonstrated ability to work within the interdisciplinary team at a senior level with effective communication.
• Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis.
• Demonstrated effective time management skills and ability to prioritise competing workloads.

Responsibilities

• Provide specialist nursing care and care coordination for patients with advanced heart failure, complex lung disease and those undergoing heart and/or lung transplantation.
• Coordinate and support patients and families throughout the transplant journey (assessment, work-up, transplantation, discharge and post-transplant follow-up).
• Work collaboratively with multidisciplinary teams to facilitate seamless patient care and optimise outcomes.
• Deliver patient and family education, participate in nurse-led clinics and monitor outcomes through PREMs and PROMs.
• Provide clinical leadership with autonomy and participate in transplant on-call services, organ retrievals and logistics.
