St Vincent’s Public Hospital in Darlinghurst, Sydney is seeking an experienced Registered Nurse to join the Heart Lung Clinic. You will provide high-quality care for heart and lung transplant, heart failure, pulmonary hypertension and related patients in a specialist outpatient setting.

In collaboration with a multidisciplinary team, you will assess, plan and coordinate patient care, deliver education and vaccinations, and assist with enrolments and referrals to optimise outcomes.

Qualifications

• Registered Nurse with AHPRA registration.
• Strong communication within an interdisciplinary team.
• Effective time management in busy, complex settings.
• Evidence of integrating theory with clinical practice.
• Commitment to ongoing professional development.

Responsabilités

• Deliver high-quality, evidence-based nursing care for heart and lung transplant, heart failure, pulmonary hypertension and related patients.
• Assess, plan, coordinate and evaluate patient care, including triage, therapies, vaccines and admissions.
• Collaborate with a multidisciplinary team to ensure safe, timely continuity of care.
• Monitor patient conditions, respond to changes, coordinate referrals to optimise outcomes.
• Contribute to service excellence through quality improvement and professional development.
