Job summary

St Vincent’s Public Hospital in Sydney is seeking a Transplant Care Coordinator, a Clinical Nurse Specialist (Grade 2) on a max-term, part-time basis (8 hours/week) until Sep 2027. The role involves specialized nursing care and coordination for heart and lung transplant patients across inpatient and outpatient settings.

You will work within the Heart & Lung Clinic, provide education, participate in nurse-led clinics, and support families through the transplant journey.

Qualifications

• Current AHPRA-registered RN (Division 1).
• Post-registration qualifications and at least three years’ experience in the specified post-graduate area.
• Demonstrated ability to work in an interdisciplinary team at a senior level with strong communication.
• Ability to manage time and priorities in a complex clinical environment.

Responsibilities

• Provide specialist nursing care and care coordination for patients with advanced heart failure, complex lung disease and those undergoing heart and/or lung transplantation.
• Coordinate and support patients and families throughout the transplant journey, including assessment, work-up, transplantation, discharge and follow-up.
• Work collaboratively with multidisciplinary teams to facilitate seamless patient care and optimize outcomes.
• Deliver patient and family education, participate in nurse-led clinics, and contribute to outcome monitoring through PREMs and PROMs.
• Provide clinical leadership and participate in transplant on-call services, organ retrievals and transplant logistics.
• Contribute to service improvement via quality initiatives, data collection, audits and research.
