Austin Health in Melbourne is seeking a TCP Project Manager to lead the development and rollout of a Nurse Practitioner-led model of care in the Transition and Restorative Care Programs. You will work with the Nurse Practitioner and the Community Aged Care Services Manager to shape nursing models and strengthen relationships with Associated Providers.

Qualifications

• Experience with project roles ideally within aged care, community health, or the broader healthcare sector.
• Excellent interpersonal and communication skills that support stakeholder management and foster collaboration
• Proficiency in project tools, including developing GANTT charts, risk registers, and executive reports.

Responsibilities

• Lead the development and implementation of a Nurse Practitioner-led model of care in our TCP Community Program.
• Work collaboratively with the Nurse Practitioner and Community Aged Care Services Manager to shape future nursing models.
• Drive design, rollout and documentation of practical nursing resources and escalation pathways for Associated Providers.
• Create a structured framework to support future in-house nursing models and a Community TCP Nurse Practitioner Model of Care.
• Manage meetings and committees; maintain project documentation and executive reports.
