Responsibilities

• Lead a quality improvement project examining engagement, referral activity and occupancy outcomes within Residential Transition Care Program (TCP) services across GV Health brokered providers.
• Collect, analyse and interpret quantitative and qualitative data to support implementation of improved service models.
• Contribute to service redesign and future planning to ensure residential TCP services remain client-centred, financially sustainable and aligned with system demand.
