Job summary

UnitingCare Queensland is seeking a casual Supported Living Facilitator to assist clients in their homes around Toowoomba. You will drive between visits, provide in-home care and support daily living activities to enhance independence.

You will be on the road for scheduled visits, performing personal care, meal prep, light housekeeping and transport to appointments. CPR/First Aid and NDIS screening are required; a valid driver license and access to a vehicle are essential.

Qualifications

• Experience in disability, aged or community services is preferred but not essential.
• Ability to drive between clients with own vehicle.

Responsibilities

• Provide in-home personal care and daily living assistance.
• Assist with meal prep, laundry, cleaning and errands.
• Transport between clients using own vehicle and schedule visits.
• Support clients to participate in community activities and appointments.

Your part in our UnitingCare team

You'll provide care to your clients living in the community, supporting their independence and improving their quality of life. With no two days ever the same, you'll spend your time on the road completing your scheduled visits, interacting one-on-one with clients and their loved ones within their homes completing activities that include:

• In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
• Must be available to drive between clients with your own vehicle
• Hours are mainly Monday to Friday between the hours of 9am-5pm
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments

What makes a difference for us ?

• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential - as long as you have positive attitude and a willingness to learn
• A current NDIS Worker screening check
• Current CPR and First Aid Certificate ( required before you start)
• Open Drivers License and access to vehicle
• Car Insurance that is in your name
