Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator for the FaDS team in Ipswich. The role focuses on assisting individuals with daily living, personal care, and autonomy within a supportive team environment.

You will collaborate with families, ensure respect and confidentiality, and contribute to individual plans that align with the person’s life aspirations. A current NDIS check and a valid driver's license are required, with regular sleepover and weekend shifts.

Qualifications

• Must be available for regular Sleepover Shifts and Weekend work.
• Current CPR and First Aid Certificate is required before you start.
• A current NDIS Worker Screening check is required.
• Open Australian Drivers License is required.

Responsibilities

• Work in partnership with the person being supported, their family and informal support networks to help achieve life aspirations.
• Provide support with everyday living situations and personal care as identified, focusing on autonomy and quality of life.
• Contribute to development, implementation and review of individual plans that reflect the person’s goals.

Your part in our UnitingCare team Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people Enable people to attain personal autonomy over their own lives and the services which they receive Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family

What makes a difference for us?

• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Must be available for regular Sleepover Shifts and Weekend work
• Being flexible (able to work varied hours and days) and reliable to meet the needs of the people we support
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn
• A current NDIS Worker Screening check
• Open Australian Drivers License
• Current CPR and First Aid Certificate (required before you start)
