Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to join our FaDS team in Oxley. You will support individuals in their daily living, focusing on choice and improved quality of life, in partnership with family and support networks.

Your role includes contributing to individual plans and promoting autonomy with respect for privacy and dignity. Flexibility to cover 24-hour rostering is needed; prior disability sector exposure is helpful but not essential.

Qualifications

• Experience working in the Disability, Aged and Community sector is ideal but not essential.
• Must be able to communicate effectively in spoken and written English.
• Current NDIS Worker Screening check is required.
• Open Australian Driver's Licence.
• Current CPR and First Aid Certificate (required before you start).

• Work in partnership with the person being supported, their family and informal support networks to help achieve life plans.
• Provide support in everyday living, including personal care, with emphasis on choice and quality of life.
• Contribute to the development, implementation and review of individual plans.
• Help people attain autonomy over their services and lives.
• Respect privacy, dignity and confidentiality of the person and family.

• Remuneration: $45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

• Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives
• Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life
• Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people
• Enable people to attain personal autonomy over their own lives and the services which they receive
• Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family

• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Great availability across 24 hour SIL rostering (Sleepovers, Morning, Afternoon and Evening shifts)
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn
• A current NDIS Worker Screening check
• Open Australian Drivers License
• Current CPR and First Aid Certificate (required before you start)
