Job summary

Claro is seeking a Support Worker to assist clients in West Tamworth, NSW, delivering personalized care and daily living supports.

You will work with families and healthcare teams to tailor plans, focus on mobility, daily tasks, and meaningful activities, while upholding dignity and safety.

Responsibilities

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision‑making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

What you’ll do (*tailored to each client as required*)
