Job summary

Claro Disability Services in Victoria is seeking a compassionate Disability Support Worker to assist clients with complex needs. You’ll work closely with families and health professionals to support mobility, daily routines, and meaningful activities.

Responsibilities

• Chat with the client (and family/advocates) to understand goals and routines.
• Help with mobility, transfers, positioning, daily tasks, and devices per the plan.
• Monitor changes, follow med/clinical plans, liaise with care team as needed.
• Provide social time and participation aligned with client interests.
• Support decision-making, respect consent, boost independence where possible.
• Respect cultural backgrounds, beliefs, and ways of being.
• Support mealtime management, manual handling, and behaviour considerations.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Monday 1330 - 2000
• Tuesday 1500 - 2300
• Wednesday 1500 - 2300
• Thursday 1500 - 2300
• Saturday 1100 - 2000
• Sunday 0700 - 1500

Week 2

• Monday 1500 - 2300
• Tuesday 1600 - 0900 (including 11pm - 7am sleepover)
• Wednesday 1730 - 1000 (including 11pm - 7am sleepover)

Total hours: 73 hours per fortnight
