Job summary

Qualifications

  • Minimum of a High School Certificate or equivalent.
  • Must hold a valid Blue Card, Yellow Card, First Aid and CPR.
  • Current Police Check issued within the past 3 months.
  • Open Driver’s Licence (international/overseas licences may be accepted for temporary visa holders).
  • Certificate III/IV in Individual Support (Disability/Aged Care) or willing to complete - proof of enrolment required.
  • Experience supporting people with disability.
  • Must be located in Hervey Bay or surrounding areas.

Responsibilities

  • Provide person-centred Community Access support in line with each client’s NDIS goals and support plan.
  • Support clients to attend appointments, social activities, shopping, recreational activities, and other community-based activities.
  • Assist clients to safely access and participate in the community while promoting independence and choice.
  • Provide transportation and/or travel assistance as required for community access and appointments.
  • Encourage clients to develop and maintain social connections and participate in activities that align with their interests and goals.
  • Provide appropriate assistance with daily living tasks where required during community-based supports.
  • Monitor and support client safety and wellbeing while in the community.
  • Maintain accurate progress notes and records in accordance with Point Care requirements.
  • Communicate effectively with clients, families, support coordinators, and other members of the care team.
  • Follow individual support plans, risk management strategies, health and safety requirements, and organisational policies.
  • Maintain professional boundaries and uphold client privacy, dignity, rights, and choice.
  • Participate in ongoing training and professional development to maintain compliance and provide high-quality support.

Position Summary

The Support Worker will provide Community Access support to clients in accordance with their individual NDIS plans. The role focuses on supporting clients to safely access the community, participate in meaningful activities, attend appointments, and maintain their independence and wellbeing.

Requirements
Key Requirements
  • Minimum of a High School Certificate or equivalent
  • Must hold a valid Blue Card, Yellow Card, First Aid and CPR
  • Current Police Check issued within the past 3 months
  • Hold a valid Open Driver’s Licence. An international/overseas driver’s licence may be accepted for eligible temporary visa holders, subject to Queensland licensing requirements
  • Have a reliable, registered vehicle with current comprehensive car insurance
  • Certificate III/IV in Individual Support (Disability, Aged Care, or related field), or willingness to work towards completion - proof of enrolment required before commencement
  • Previous paid or lived experience supporting people with disability
  • Must be currently located in Hervey Bay or surrounding areas at the time of application
  • Experience providing Community Access support, including assisting clients with appointments, outings, shopping, social activities, and community participation
  • Strong understanding of person-centred support, client choice, independence, and NDIS goals
  • Strong understanding of the NDIS and its Practice Standards
  • Sound knowledge of health and safety requirements and duty of care obligations
  • Ability to support a diverse range of clients with different abilities, backgrounds, communication styles, and support needs
  • Strong communication and interpersonal skills, with the ability to build positive and professional relationships with clients and their support networks
  • Good organisational skills and the ability to maintain accurate progress notes and support records
  • Basic computer literacy, including the ability to use Microsoft Office and digital support systems
  • Willingness to complete ongoing mandatory training and professional development as required by Point Care
