Job summary

Planet Support Services is seeking an experienced Support Worker to assist participants with community participation, skill development and independent living in the Hervey Bay area. You will support with transport, daily tasks and goal building while fostering confidence and engagement.

The role offers stable hours (approximately 10-20 hours per week), a supportive management team and a friendly workplace focused on genuine client relationships within the local community.

Qualifications

• Experience as a Disability Support Worker is preferred.
• Self-motivated, reliable and able to work independently.
• Proactive attitude with initiative.
• Passion for helping people achieve personal goals.
• Strong communication and interpersonal skills.
• Current driver licence; First Aid & CPR or willingness to obtain.
• NDIS Worker Screening Check and Working with Children Check or willingness to obtain.

Responsibilities

• Support participants to access community activities, appointments and outings.
• Provide transport to appointments, shopping and recreational activities.
• Assist with household tasks including cooking, cleaning and laundry.
• Encourage confidence, independence and life skills development.
• Identify opportunities for new activities and community engagement.
• Provide occasional mild personal care within scope of practice.
• Build positive, professional relationships with participants and their networks.
• Work within training, experience and scope of practice.

We're looking for an experienced and motivated Support Worker who enjoys working independently and takes pride in making a genuine difference in someone's life.

This role focuses primarily on community participation, skill development and supporting independence and in home assistance. We're looking for someone who enjoys helping others achieve their goals, thinks proactively and is confident taking initiative to create engaging and meaningful supports.

Your role will include:

• Supporting participants to access their local community, attend activities, appointments and outings
• Providing transport to appointments, shopping and recreational activities
• Assisting with everyday household tasks including cooking, cleaning, laundry and general organisation
• Encouraging participants to build confidence, independence and life skills
• Identifying opportunities for new activities, hobbies and community engagement
• Providing occasional mild personal care where required
• Building positive, professional relationships with participants and their support network
• Working within your training, experience and scope of practice

• Previous experience as a Disability Support Worker (preferred)
