Job summary

UnitingCare Wesley Bowden in Adelaide seeks a Support Coordinator to provide person-centred NDIS support coordination for participants of all ages. You will help clients understand their plans, connect with providers, and support them to manage their supports with confidence.

You will work with participants, families and providers in a do-with, not-for approach, ensuring safe plan implementation and cultural safety in diverse communities.

Job description

As a Support Coordinator you will provide person centred NDIS support coordination to participants of all ages and backgrounds. Your role will help people understand and use their NDIS plans, connect with suitable providers and community supports and build confidence in managing their own supports over time.

Working from a “do with, not for” approach you will support participants to make informed choices and strengthen their capacity to work towards their goals. You will work with participants, families, carers and providers to support safe, practical and effective plan implementation.

To thrive in this role you will be a compassionate and organised professional who understands the importance of person centred support, choice and inclusion. You communicate clearly and respectfully and you can build trust with participants, families, carers and providers.

You manage your time well, keep your work organised and can adjust when priorities change. You are confident working independently, while also knowing when to seek guidance or work with others to support good outcomes.

You bring a strong commitment to cultural safety and inclusion, especially when working with diverse communities, including LGBTIQA+ people and Aboriginal and Torres Strait Islander peoples.

You are confident using Microsoft Office and cloud based systems to document your work and report on the services you provide. You understand the importance of accurate records, clear communication and following organisational processes.

You work with integrity and transparency, building positive relationships with participants, service providers and community organisations. You care about helping people build confidence, make informed choices and work towards greater independence.

You will have:
  • A South Australian Driver’s Licence
  • Current DHS Working with Children Check (WWCC)
  • Current Police Clearence
  • NDIS Worker Screening Check
