We are seeking an experienced Support At Home (SAH) Administrator to provides high-quality administrative, coordination and customer service support to the Care Partner team.
The Support At Home (SAH) Administrator ensures efficient day-to-day operations by managing participant records, scheduling appointments, maintaining documentation, supporting care planning processes, and facilitating communication between participants, families, Care Partners and internal stakeholders.
This position plays a vital role in ensuring older people receive responsive, person-centred support and services.
Prepare, process and maintain participant records, correspondence and documentation.
Manage incoming phone calls, emails and enquiries, responding appropriately or directing to relevant team members.
Maintain accurate and up-to-date participant information within organisational systems.
Assist with onboarding new participants, including collection and processing of required documentation.
Support the preparation of service agreements, care plans and review documentation.
Ensure participant files meet organisational, legislative and regulatory requirements.
Ensure documentation is managed in accordance with Aged Care Standards, privacy legislation and organisational policies.
