Northern Health in Melbourne's rapidly growing outer north is seeking a motivated Grade 1 Social Worker to join the Subacute Social Work team on a fixed-term, full-time basis until January 2027. The role involves conducting comprehensive psychosocial assessments and delivering patient-centered care across multiple sites, with opportunities for education and supervision within a multidisciplinary team.

Qualifications

• Significant experience undertaking comprehensive psychosocial assessments and interventions.
• Significant experience practicing patient centered care.
• Demonstrated experience prioritising and independently managing complex social situations.
• Well established effective verbal and written communication skills.
• Demonstrated ability and commitment to work in a multi-disciplinary team.
• Demonstrated commitment shown to ongoing professional development.
• Dedication to and demonstrated involvement in Quality Improvement.
• Experience in education, support and supervision to Grade 1 Social Workers and/or students.
• Experience working with the cultural and socioeconomic diversity of our community.

Responsibilities

• The Grade 1 Social Worker will provide quality patient care through assessment, planning, implementation of treatment and management.
• Will work across multiple subacute teams and sites, adapting to different clinical environments.
