Job summary

Bayside Health is seeking an experienced Grade 1 Social Worker to join our Regional Care Group in Leongatha and Korumburra, Victoria. The role delivers person-centred psychosocial care, with assessments, clinical reasoning, and discharge planning across the lifespan.

The position may span acute, subacute, medical and community social work settings, with supervision and involvement in quality improvement initiatives. Applicants should hold a qualifying social work degree and relevant checks.

Qualifications

• Bachelor or Master of Social Work with eligibility for AASW membership.
• Current Police Check, Working with Children Check and NDIS Worker Screening Check.
• Understanding of child safeguarding and family violence requirements (including MARAM).

Responsibilities

• Conduct multidimensional psychosocial assessments and formulate complex intervention and discharge plans.
• Provide safe, person-centred care in partnership with clients, families and healthcare teams.
• Work across admitted and community social work services with regular supervision and quality-improvement participation.
