Job summary

Northern Health in Melbourne’s rapidly growing northern suburbs is seeking a Grade 1 Social Worker to join the Social Work team in a rotating, inpatient ward role at The Northern Hospital Epping. The role provides patient-centred care through psychosocial assessments, discharge planning and collaborative interventions within a multidisciplinary team.

This is a permanent full-time position with 6-month rotations, opportunities for supervision and professional development, and a strong focus on

Qualifications

• Experience in undertaking psychosocial assessments and putting appropriate interventions in place.
• Proven ability to address stressful clinical and/or non-clinical situations appropriately.
• Demonstrated clinical skills at a new graduate Grade 1 level of competence.
• Well-developed written communication skills as evidenced in documentation and reports.

Responsibilities

• Provide quality patient-centred care through assessment, planning, implementation of treatment and discharge planning.
• Rotate into various clinical roles after fixed periods (currently 6 months).
• Respond to inpatient referrals and provide psychosocial assessments and interventions to patients and families.
• Work as part of a multidisciplinary team on inpatient wards.
